If you’re traveling from out of town, our offices are located in Chicago’s Gold Coast neighborhood. The Gold Coast is close to some of the city’s most iconic attractions and striking architecture, as well as a host of dining, shopping and nightlife.
Your procedure may require recovery time during which it may not be advisable to travel. This period may also include follow-up visits to ensure you’re healing and recovering properly. For these reasons we advise out-of-town patients to consider a lengthier stay in the Chicago area.

To begin your treatment process, we require a $100 consultation fee. This covers your initial appointment and will be applied to the total cost of your treatment. Once we determine a complete course of treatment, we’ll arrange a payment schedule for the balance of the fees.

Below are the two options for patient parking at the Center:
900 Self Park:
900 Self Park is located in the same building as the Evolve Your Life, Inc., 60 East Delaware Clinic and can be entered from Walton (traveling westbound, away from Lake Michigan) or Rush Street (traveling north).
For easier access to the Evolve Your Life, Inc., 60 East Delaware Clinic, park near the Delaware side elevators (south elevators) so you may take the elevator directly to the 14th floor; should you park on the Walton side elevators you will need to change elevators on level 8, cross through the mall corridor and take the Delaware elevators to level 14. Please go down to the suite at the end of the long corridor, and you will find Suite 1430, Evolve Your Life, Inc.
All patients are entitled to a validated 3 hour parking ticket; one ticket per patient.
Entry to this parking lot before 10AM and exit by 7PM qualifies the customer for the Early Bird discount (3 hour parking validation plus $1.00 ($1 to be paid by customer).
NCR Parking:
Alternatively, you can use the valet parking at NCR Parking at Delaware Place and Ernst Ct, which may offer better rates for customers parking longer than 3 hours. To access this parking you should be traveling east (towards Lake Michigan and Michigan Avenue) on Delaware Place then take a left into Ernst Ct (alley between 900 N Michigan building and building next to it) immediately after you pass the 900 Self Park structure at 60 East Delaware Place.
