What Is Gynecomastia Surgery and How Is Male Breast Tissue Removed?
QUICK ANSWER
Gynecomastia surgery removes excess glandular tissue and/or fat from the male chest to create a flat, masculine contour. It is performed under general anesthesia as an outpatient procedure using liposuction alone, direct excision, or a combination of both — depending on whether the enlargement is primarily fatty or glandular.
What Is Gynecomastia?
Gynecomastia is the medical term for the enlargement of breast tissue in men. It affects an estimated 40 to 60 percent of men at some point in their lives. It may involve one or both breasts and can cause physical discomfort, tenderness, and significant psychological distress.
True gynecomastia involves proliferation of glandular breast tissue. Pseudogynecomastia — which appears similar — is caused by fat accumulation without glandular growth. The distinction is important because the two require different treatments.
What Causes Gynecomastia?
- Hormonal imbalance between estrogen and testosterone — most common in puberty and older age
- Anabolic steroid use
- Certain medications (spironolactone, some antidepressants, proton pump inhibitors)
- Obesity — which increases aromatization of testosterone to estrogen
- Liver disease, kidney failure, or hypogonadism
- Idiopathic (no identifiable cause) — the most common category in adult men
Who Is a Good Candidate for Gynecomastia Surgery?
The best surgical candidates for gynecomastia correction are men whose chest enlargement has been stable for at least six months, who have tried conservative measures (weight loss, addressing medications), whose condition is not expected to resolve with treatment of an underlying cause, and who are in good overall health.
How Is Gynecomastia Surgery Performed?
Liposuction alone (pseudogynecomastia or mild true gynecomastia)
When the excess tissue is primarily fat — or when mild glandular tissue is present — liposuction alone may achieve an excellent result. Small incisions (3 to 5 mm) are placed near the armpit or the areola edge. A cannula is used to suction out the excess fat and, in mild cases, softer glandular tissue.
Glandular tissue excision (true gynecomastia)
When firm glandular tissue is present — often felt as a disk-like mass beneath the areola — surgical excision is required. A small periareolar incision (along the lower edge of the areola) provides access to remove the tissue directly. Scarring is minimal and well-concealed at the border of the areola.
Combined liposuction and excision
Most patients with moderate to severe gynecomastia have both fatty and glandular components. Combining both techniques produces the most thorough and natural-looking result. This is the most common approach used by Dr. Dorafshar at Evolve Your Life.
What Is Gynecomastia Surgery Recovery Like?
- Days 1–3: Soreness and bruising. Compression vest worn.
- Week 1–2: Desk work may resume. No lifting or strenuous activity.
- Week 4–6: Exercise and full activity typically cleared.
- 3–6 months: Final contour visible as swelling fully resolves.
Does Insurance Cover Gynecomastia Surgery?
Insurance may cover gynecomastia surgery when the condition is linked to a documented medical cause (such as a hormonal disorder or medication side effect) and causes functional symptoms. Coverage for purely cosmetic gynecomastia is generally not available. The Evolve Your Life team can assist patients in submitting documentation to support coverage requests.
Frequently Asked Questions
Will gynecomastia return after surgery?
In the vast majority of cases, gynecomastia does not return after surgery. Avoiding anabolic steroids and maintaining a healthy weight significantly reduces the risk of recurrence. If an underlying hormonal condition was treated, recurrence is further minimized.
Are gynecomastia surgery scars visible?
Scars are minimal. Periareolar incisions heal at the border of the areola — an area where skin color and texture naturally camouflage scarring. Most patients find scars barely noticeable after 12 months.
Is gynecomastia the same as excess chest fat from obesity?
Not always. Pseudogynecomastia is caused by fat accumulation and resolves significantly with weight loss. True gynecomastia involves glandular tissue that does not respond to diet or exercise and requires surgical removal.
How long does gynecomastia surgery take?
Depending on the technique, gynecomastia surgery takes one to two hours and is typically performed as an outpatient procedure under general anesthesia or IV sedation with local anesthesia.
Can gynecomastia surgery be part of a daddy do-over?
Yes. Gynecomastia correction is one of the most commonly requested components of a daddy do-over, often combined with abdominal liposuction or a tummy tuck.
