This lady presented to Dr Dorafshar following a previous breast augmentation procedure by another surgeon. She was dissatisfied with the cosmetic appearance of the breasts and had “bottoming out” of the implants and effacement of her inframammary fold. Dr. Dorafshar reconstructed the inframammary fold by placing a mesh to support the implant and switched out the implants to a more projected type to give her a natural, beautiful result.A patient sought consultation with Dr. Dorafshar after undergoing a prior breast augmentation performed by a different surgical provider. Her primary concerns revolved around the unsatisfactory aesthetic outcome of the initial procedure. Specifically, she exhibited a complication known as “bottoming out,” where the breast implants had migrated inferiorly, and there was a noticeable flattening or disappearance (effacement) of the natural crease beneath the breast, known as the inframammary fold. To address these issues and achieve the patient’s cosmetic goals, Dr. Dorafshar undertook a revision breast augmentation procedure. A key aspect of this revision surgery involved the meticulous reconstruction of the inframammary fold. To provide durable support for the breast implant in its corrected position and prevent future bottoming out, Dr. Dorafshar strategically placed a surgical mesh. Furthermore, recognizing that the existing implants contributed to the undesirable shape, Dr. Dorafshar replaced them with a new set of implants featuring a more projected profile. This change in implant type was intended to create a more natural and aesthetically pleasing breast contour, ultimately delivering the beautiful result the patient desired.
