The initial surgical stage focused on the periorbital region and the chin/jawline. A lower transconjunctival blepharoplasty was meticulously performed, involving a conservative resection of excess fat and strategic redraping of the remaining fat over the orbital rim. This technique aimed to effectively improve the tear trough deformity, a common concern associated with aging in the lower eyelids. Concurrently, an osseous genioplasty was undertaken to advance the chin. This procedure was designed to enhance the patient’s jawline definition and improve the contour of her neck line, ultimately contributing to a more balanced and youthful facial profile. The genioplasty was executed via an intraoral approach, ensuring that all incisions and subsequent scars would be concealed within the mouth. To further refine facial contours and address volume loss, liposculpture techniques were employed to remove unwanted fat deposits, and the harvested fat was then strategically grafted to areas requiring augmentation.
The second surgical stage involved a deep plane face lift, a technique known for its ability to address deeper facial structures and provide a more significant and long-lasting rejuvenation effect. This procedure was complemented by a submental incision to perform an anterior platysmaplasty, which tightens the platysma muscle in the neck to further improve the neck line and contour. Additional fat grafting was performed during this stage to optimize the distribution of fat in the face, replenishing volume loss that occurs with age and contributing to a fuller, more youthful appearance. Furthermore, the benefits of fat grafting extend beyond volume restoration, as the stem cells present in the grafted fat can contribute to improved skin quality and texture.
