This patient presented with concerns regarding the appearance of tired and aged eyes, specifically expressing dissatisfaction with fullness in her upper eyelids. To address these concerns and rejuvenate the periorbital region, Dr. Dorafshar performed a comprehensive surgical approach consisting of a brow lift, upper blepharoplasty, and lower blepharoplasty.
The brow lift aimed to elevate and reposition the eyebrows to a more youthful and natural arch, which can indirectly improve the appearance of the upper eyelids. The upper blepharoplasty procedure focused on removing excess skin and fat from the upper eyelids, directly addressing the patient’s complaint of fullness. Dr. Dorafshar employed specialized techniques during the upper blepharoplasty to directly thin the subcutaneous tissue beneath the eyebrows, further contributing to a reduction in upper eyelid fullness and creating a smoother contour.
The lower blepharoplasty was performed utilizing a combined approach to address multiple concerns of the lower periorbital area. A transconjunctival approach, where the incision is made inside the lower eyelid, was used to improve the appearance of the tear troughs without any visible external scar. Simultaneously, a subciliary approach, involving a delicate incision just below the lash line, allowed for the removal of excess skin from the lower eyelids. To restore lost volume and further enhance the rejuvenation, autologous fat grafting was performed, where the patient’s own fat was harvested, processed, and carefully injected into targeted areas around the eyes, such as the cheeks or tear troughs, to provide a more youthful and refreshed appearance. The combination of these techniques aimed to create a harmonious and natural-looking rejuvenation of the entire periorbital area, addressing the patient’s initial concerns of tired and aged eyes.
