Facial Feminization Surgery Patient – Chicago, IL
A transfeminine patient, after extensive research to find a surgeon specializing in transgender surgery, discovered Dr. Dorafshar’s practice. Impressed by his reputation and expertise, she scheduled several consultations to discuss her goals for facial feminization surgery. During these detailed discussions, Dr. Dorafshar meticulously outlined the surgical plan, addressing her concerns and expectations. Feeling confident and well-informed, she made the decision to proceed with the comprehensive facial feminization procedure.
The surgery was meticulously performed and encompassed three key areas of the face:
- Upper Third of the Face: Through a carefully executed hairline incision, the frontal bone was exposed. Dr. Dorafshar performed a precise frontal sinus setback and meticulous remodeling of the supero-orbital rims, aiming to achieve a softer, more feminine forehead contour. Debulking of the forehead muscles was also performed to reduce prominence. Additionally, a brow ptosis repair and scalp advancement were carried out to lift the brows and achieve a harmonious hairline.
- Middle Third of the Face: To enhance facial volume and create a more youthful and feminine appearance, fat grafting of the midface was performed. Special attention was given to the malar (cheekbone) areas, adding subtle projection and contour. Further fat grafting was strategically applied to the upper and lower lip regions, contributing to a fuller, more sensuous lip shape.
- Lower Third of the Face: Addressing the lower face, Dr. Dorafshar utilized gingivolabial incisions, carefully placed within the mouth to avoid external scarring. Mandibular contouring was performed, specifically targeting the gonial angles (jawline corners) to create a softer, less angular jawline. This was complemented by a three-piece genioplasty, a sophisticated technique used to reshape the chin for a more delicate and feminine profile.
In the postoperative period, the patient expressed profound satisfaction with the results. She was particularly delighted with how her eyes “opened up,” a transformative change that significantly contributed to her feeling more congruent with her gender identity. The aesthetic refinements allowed her to “pass” more seamlessly in public, experiencing a marked decrease in instances of her gender being questioned. This newfound sense of acceptance and alignment with her true self greatly enhanced her quality of life and self-confidence.
