Facial Feminization Surgery Patient – Chicago, IL
This detailed case study outlines the journey of a transfeminine patient who sought facial feminization surgery with Dr. Dorafshar to address her gender dysphoria. Following a comprehensive consultation, the patient decided to proceed with a multi-stage surgical plan designed to feminize various regions of her face and neck.
The upper facial third surgery focused on creating a softer, more traditionally feminine forehead and eye area. This included a frontal sinus setback, which involved reshaping the bone of the forehead to reduce its prominence, and contouring of her superior orbital rims to create a smoother, less angular brow. Additionally, brow elevation was performed to reposition the eyebrows into a more feminine arch, and a scalp advancement procedure helped to reduce the forehead height and create a more harmonious hairline.
For the middle third of the face, the patient underwent fat grafting. This technique involved harvesting fat from another part of the body and carefully injecting it into the midface region. This procedure aimed to add volume and create a more rounded, softer contour to the cheeks, contributing to a more youthful and feminine appearance.
The lower third of the face was addressed with two key procedures. Genioplasty, or chin surgery, was performed to reshape and refine the chin, making it appear softer and more feminine, often involving a reduction in its projection or width. Mandibular contouring focused on reshaping the jawline, typically reducing the angularity and width of the jaw to create a more delicate and feminine lower facial silhouette.
Finally, the neck area was addressed with a tracheal shave, also known as a chondrolaryngoplasty. This procedure involved reducing the prominence of the thyroid cartilage, often referred to as the “Adam’s apple,” to create a smoother, more feminine neck contour.
Post-operatively, the patient’s transformation was remarkable. Photographs clearly illustrate the significant feminization achieved, particularly in the orbital region, where her eyes now appear more open and her brow softer. The results also show a noticeable softening of her chin, contributing to an overall more feminine facial profile. The patient expressed immense satisfaction with her results, reporting a profound increase in her confidence regarding her gender identity, which was the ultimate goal of the comprehensive facial feminization process.
