The surgical procedure meticulously addressed various facial regions to achieve a harmonious and feminized outcome:
Upper Face: Access was gained through a discreetly placed hairline incision, ensuring minimal visible scarring. The reconstructive work in this area involved a multi-faceted approach. A frontal sinus setback was performed to reduce the prominence of the brow bone. This was complemented by supraorbital remodeling, further softening the contours around the eyes. Forehead bone reshaping was carefully executed to create a smoother, more rounded forehead. A brow lift was performed to elevate the eyebrows, opening up the eyes and creating a more feminine arch. Additionally, a differential scalp advancement was utilized to correct an M-shaped hairline, creating a softer and more natural hairline that frames the face.
Middle Face: Enhancements in this region focused on adding volume and refining features. Fat grafting was strategically applied to the malar regions (cheekbones) to create fuller, more youthful, and contoured cheeks. Similar fat grafting techniques were employed for both the upper and lower lips, increasing their volume and improving their definition. A closed rhinoplasty was performed to reshape the nose without external incisions, refining its overall appearance. Weir excisions were also performed to reduce the width of the nostrils, contributing to a more delicate nasal structure.
Lower Face: The surgical approach to the lower face was carried out through hidden gingivolabial incisions, located inside the mouth, to avoid external scarring. A multi-piece genioplasty was performed to reshape and reposition the chin, achieving a more graceful and balanced profile. Concurrently, a mandibular gonial angle reduction was performed to soften the angles of the lower jaw, creating a more oval or heart-shaped facial contour.
Post-operatively, the patient experienced a profound sense of satisfaction and elation. Her feminized face now beautifully aligned with her gender identity, bringing her a deeply desired sense of congruence and contentment.
