A septuagenarian female presented with significant weight loss, resulting in a noticeably deflated and aged facial appearance characterized by substantial volume depletion. To address these concerns, Dr. Dorafshar undertook a comprehensive, multi-stage facial rejuvenation. The initial surgical phase involved a deep plane face and neck lift, accompanied by an upper lip lift. Additionally, liposculpture techniques were employed to refine the lower facial contours, and fat grafting was utilized to restore lost volume in key areas, thereby improving facial fullness and youthful contours. In a subsequent stage, a brow lift was performed to elevate the eyebrows and address forehead wrinkles, and a four-lid blepharoplasty was executed to rejuvenate the upper and lower eyelids. Given the patient’s considerable excess lower eyelid skin requiring both tightening and excision, a transcutaneous, or traditional, lower lid blepharoplasty approach was deemed most appropriate. Following the completion of these procedures, the patient exhibited a remarkably pleasing aesthetic outcome and expressed profound satisfaction with the overall results of her facial rejuvenation journey.
