A female patient of Hispanic descent presented seeking rhinoplasty to address the characteristics of her Mestizo nose. Her primary aesthetic goals were to reduce the prominence of her dorsal hump and achieve a subtle upward rotation of the nasal tip, while ensuring the outcome remained harmonious with her ethnic features. An open rhinoplasty technique was employed to provide optimal visualization and control over the nasal structures. The surgical plan involved meticulous reduction of the cartilaginous and bony components of the dorsal hump to create a smoother nasal profile. The nasal tip was refined and its projection and rotation were carefully augmented through suturing techniques and conservative cartilage grafting, aiming for a natural and ethnically appropriate upturn. Postoperatively, the patient exhibited a significant improvement in the nasal profile with a noticeable reduction of the dorsal hump and a more refined, slightly elevated nasal tip. The final result successfully achieved the patient’s objectives of maintaining her ethnic identity while attaining a more balanced and aesthetically pleasing nasal appearance. The open approach facilitated the precise modifications necessary to deliver a natural and ethnically congruent outcome that addressed the patient’s specific desires for a less prominent dorsal hump and a subtly more upturned nasal tip.
