This young lady presented to our clinic with a dual concern: primarily, significant difficulty breathing through her nose, and secondarily, aesthetic dissatisfaction with what she described as a “bulbous nasal tip.” These issues significantly impacted her quality of life, both functionally and cosmetically.
Following a thorough consultation and in-depth discussion with Dr. Dorafshar, a comprehensive surgical plan was formulated. The chosen approach was an open septorhinoplasty, a technique that allows for optimal visualization and precise manipulation of the nasal structures.
During the procedure, several critical steps were undertaken to address both her functional and aesthetic concerns:
- Nasal Dorsum Reduction: Her nasal dorsum, or bridge of the nose, was carefully reduced to achieve a more harmonious profile and balance with the rest of her facial features.
- Septoplasty for Airway Improvement: A septoplasty was meticulously performed to correct a deviated septum, a common anatomical obstruction that was contributing to her breathing difficulties. This crucial step was aimed at significantly improving her nasal airway and airflow.
- Spreader Cartilage Grafts: Spreader grafts, typically harvested from the patient’s own septal cartilage, were meticulously fashioned and placed. These grafts are instrumental in maintaining the patency of the middle vault of the nose, preventing collapse, and further enhancing airflow.
- Tip Rotation and Refinement: Her nasal tip, which was the source of her aesthetic concern, was rotated upwards to achieve a more refined and youthful appearance.
- Tip Defining Sutures: To specifically address the “bulbous” nature of her nasal tip, precise tip defining sutures were strategically placed. These sutures helped to narrow and sculpt the tip, creating a more defined and aesthetically pleasing contour.
Post-operatively, the patient experienced a remarkable transformation. She expressed immense delight with her new aesthetic, noting a significant improvement in the overall balance and refinement of her nose. Crucially, and perhaps most importantly for her quality of life, she could finally breathe better, indicating the successful resolution of her primary functional concern. This case exemplifies the successful integration of functional and aesthetic goals in rhinoplasty, resulting in a highly satisfied patient.
